The District will use the following criteria to identify (1) children in Pre-K through grade 4 who are targeted for individualized prevention strategies under this plan; and (2) children who are specifically considered “at risk” in grades 5 through 12. These criteria reflect a combination of mandatory criteria established under state law (sec. 118.153 and ch. PI 25) and additional locally-selected criteria:

  • Any student who is identified as a school dropout as defined in PI 25, including any student who has ceased attending school for 10 or more consecutive school days without an excused absence; or
  • Any student who meets 2 or more of the following criteria:
    • Habitual Truant (5 days or more of unexcused absences within a school semester)
    • Two or more years behind their age group in basic skill levels, based on a plan of assessment that is consistent with the District’s standards and objectives in math and reading as identified in the applicable curriculum plan.
    • Not on track for graduation based on the graduation policy required credits.
    • School-age parent
    • Adjudicated delinquent
    • AODA use/abuse
    • Documented mental health issues
    • Eighth-grade pupils whose score in each subject area on the State-required examination was below the basic level and eighth-grade pupils who failed a majority of middle school courses.
    • Documented concerning behavior patterns with approval of Selection Committee.

The Pupil Services Team (nurse, administration, counselors, police school resource officers, school psychologists, at-risk coordinator, director of pupil services) is responsible for identifying, providing services, and programming for students at-risk. The Building Pupil Services Teams meet weekly throughout the year in order to identify struggling students. Placement would happen at the beginning of each term, upon new student registration, and then again from the weekly meetings depending upon individual circumstances. Parents and students may also make a direct inquiry regarding the services identified in this plan, and the team will assess the self-referral. 

Student services are available for everyone in the school. These services include assistance with educational planning, interpretation of test scores, occupational information, career information, study skills, help with home, school, and/or social concerns, or any question the student may feel he would like to discuss with the counselor.

Students may be offered the following prevention strategies to prevent students from becoming at-risk:

  • Collaboration with staff and families
  • Classroom Guidance Instruction
  • Small group counseling
  • Individual counseling
  • English Language Learner (ELL) Instruction
  • New Student Orientation
  • Parent Peer Advocate
  • Summer School

Specific to pre-Kindergarten-4:

  • Title I (K-8)
  • 4 year old kindergarten
  • Second Step Curriculum

Specific to Intermediate/Middle/High School:

  • Title I (K-8)
  • Prevention efforts for bullying, suicide, and AODA
  • Sources of Strength
  • ACP Coordination
  • Post-Secondary Planning

Parent/guardian(s) will be informed and involved in the programs or services made available for grades pre-kindergarten-12 through the Pupil Service Team and contribute to the creation of a student plan. 

The Pupil Services Team will make clear what services are available and how the student may participate.

Accommodations that may be made to support individual student achievement and success in school:

  • Alternative Education Program/Credit Recovery
  • Online courses Edgenuity (5-12)
  • School to work/youth Apprenticeship
  • Adult and Peer mentoring
  • Homebound
  • Special Education for identified students
  • Section 504 for identified students
  • Enforcement of Attendance
  • Alternative Scheduling
  • Intervention in Math/Reading
  • PATH Counseling (5-12)
  • TRIUMPH Education Option (9-12)  

The Pupil Services Team will evaluate and report to the school board annually the success of the services provided under the at-risk plan by:

  • Increase in graduation rates
  • Decrease in dropout rates
  • Improved school attendance/link to AYP 85%
  • Survey population (parent and child)
  • Decrease in legal referrals
  • Decrease in disciplinary referrals
  • Decrease in course failures

The staff will be informed about the at-risk plan and available services through:

  • Committee
  • In-service
  • Process of student identification
  • Curriculum collaboration
  • Staff Meetings
  • District Web Page

Community partners will be informed about at-risk services through:

  • Information provided on the District Website
