ALL registration materials are available below and you MUST download, complete and bring this information with you to registration. You may need to download the most current Adobe.pdf software to complete the forms. To do this, go to http://www.adobe.com/products/reader/html . If you are a Google Chrome user some forms may try to download instead of automatically open. There will be a limited number of computers and a printer available at registration if you lose, forget, or do not complete the forms that are on line. If you do not have internet access from home, all public libraries have computers available for free use. Failure to complete the online forms will likely double your wait time during registration.
When you complete the registration process, you will be informed of your child’s homeroom teacher. This information will not be given out by telephone prior to the registration process.

Medical Alert List Update Request
Request for Giving Medicine at School - required only for students receiving medication during the school day
Consent for Self-Administration of Medication - required only for students who have parental consent to self-administer medication while at school.
Network Photography Orders—IF PURCHASING PICTURES-- Note: All students are required to have their picture taken for the yearbook even if they do not purchase a picture packet.
Free and Reduced Meal Application Form—IF APPLICABLE (only need to print pages 5-7, one per family)
